What to bring to your GLP-1 follow-up
One page, six things: the doses you took and when, any missed or late doses, your weight by week, side effects and when they happened, the spots you injected, and your questions.
Not medical advice. Your prescriber decides your treatment; this page only helps you bring the right notes.

The checklist
- The doses you took, and whenEach dose, its date, and the amount you took.
- Missed or late dosesWhich ones, and how late. Everyone misses one sometimes, and knowing helps your prescriber read the rest.
- Your weight, by weekOne number a week, taken the same way each time, shows the trend better than daily ups and downs. If you are losing faster than about 1% of your body weight a week, say so.
- Side effects, and when they happenedWhat it was, which day it started compared with your dose day, how long it lasted and how strong it felt.
- Injection sites and any reactionsWhere you injected, and any redness, itching, swelling or lumps afterwards.
- Anything else that changedNew medicines or supplements, an illness, travel, your appetite or your sleep.
Questions to ask
- Is my progress about what you expected by now?
- Which side effects should make me call you before the next visit?
- What should I do if I miss a dose or take it late?
- What is the plan for my dose from here?
- Should I be eating or drinking more of anything, like protein or water?
- When should we talk next?

PeptideUp puts it on one page
The prescriber page in PeptideUp shows your last 12 weeks on one page: your weight trend, the doses you logged against your schedule, the dose steps you logged, your recent doses with the spot you used, and the side effects you noted. Share it as a PDF or print it.
It is made on your iPhone and goes only where you send it. It shows what you logged; PeptideUp never suggests a dose.
Questions people ask
Do I need to bring daily weights?
No. One weight a week, taken the same way each time, shows the trend more clearly. Bring daily numbers only if your prescriber asks for them.
Should I mention doses I missed?
Yes. Missed and late doses are common, and knowing about them helps your prescriber make sense of your results.
Can I show it on my phone?
Yes, or print it. One page is the easiest thing to read in a short visit.
PeptideUp is a tracker, not medical advice. It never recommends a medicine, a dose or a schedule.
